Medical Records

Medical Records and Billing Records can be submitted with the facility name “Midwest Neuroscience Institute/Center for Relief of Pain” for proper processing. For Medical Records Requests, FMLA or Short-Term Disability paperwork, please call 816-363-2500 and ask to speak to Medical Records. Medical Record Request can also be faxed to 816-361-4500.

Prescriptions

We take care of prescription refills only during office hours. Please call your pharmacy to ask for a refill, and the pharmacy will then contact us. New Patients

We generally see new patients on referral by their primary care physician or another specialist.
