Our office is open from 9 a.m. To 5 p.m. Monday through Friday.
There is always a physician on call 24 hours a day to assist in emergent care of our patients.
We participate in most insurance plans.
We take care of prescription refills only during office hours. Please call your pharmacy to ask for a refill, and the pharmacy will then contact us. Make sure you allow at least 48 hours to receive your refill. Controlled substances require a written prescription. Please call our office during office hours three days before your need a refill.
We generally see new patients on referral by their primary care physician or another specialist.
We accept most major insurance. If you do not see your insurance listed below, please contact our offices to determine if we honor your carrier.

What do I need to bring to my new patient appointment?
Your imaging CDs and reports
Completed health history and demographic forms
Referral from your referring or primary care physician (if applicable)
Driver’s license or other government issued identification card
Cash, check or credit card to pay for your visit or your co-pay/co-insurance payment (if applicable)
If you are filing a worker’s compensation claim, we will need the name, address, and telephone number of your worker’s compensation carrier in addition to their initial authorization.
If you are being seen for any auto related injury, please be sure to bring all auto claim information.

How do I request a medication refill?
Contact your pharmacy and request a refill. They will contact our office for you with all the pertinent information. Please allow up to 2 business days for a refill. Controlled substance prescriptions require a written prescription. Please call our office three days before you need your refill.
What happens to my MRI or X-Ray films once I’ve brought them to the office?
We encourage all patients to personally bring a CD of their imaging to their appointment. Imaging CDs that arrive prior to the appointment are logged into a computer database, filed and pulled for your appointment.
What happens to my films after I’ve had surgery?
They are retained for any possible surgical options, or can be returned to you upon request.
